FIDANTE Bonus Exchange Plan Election and Authorisation Form

This is my request to participate in the Bonus Exchange Plan:

Bonus Exchange

I wish to receive £ of my bonus payment as a contribution to the Fidante Workplace Pension Plan (with AEGON). I understand that this payment will be increased by 13.8%. Any excess bonus over the amount exchanged is to be paid through the payroll in the normal way.

Investment Choice

I understand that my contribution will be invested in the same funds as my monthly contributions.

3.

Declaration

I authorise Fidante to reduce my bonus payment, as per my instructions above. I understand that the equivalent amount will be paid towards my pension (plus the 13.8% enhancement).

I am aware that Fidante recommends that anyone thinking of participating in the Bonus Exchange plan should take financial advice beforehand. I understand that I am responsible for ensuring that any additional Pension contribution made using Bonus Exchange falls within the applicable HMRC limits.

I confirm that I am fully responsible for my decision to participate in the Bonus Exchange Plan.
